Little is known about Earth-2772 Strange's early life, but he had a younger sister named April Strange of who he clearly loved, even though one teased another on daily basis. In one of those days, after Strange forcefully applied a crude lipstick on his sister and she assaults him, she suddenly fainted and Stephen promptly take her to hospital. After an exam, she was diagnosed with a fatal, incurable disease and the doctor warned him that's a mere matter of time before she dies. Already a famed physician and distressed over the news, Stephen refused to accept the diagnosis and decided to be April's surgeon (even though, in real life, physicians are not allowed to treat family menbers, implying that Strange was disrespecting ethical rules). At the day of the surgery, Stephen promised to April that the operation would be a success and all would end well for them. Unfortunately, April died at the operation and, although Strange's skills at medicine would greatly improve in time and grant him great fame and respect, Stephen come to blame himself over his sister's death and would grow greedy and arrogant in a matter of years (possibly a coping mechanism of his guilty over April's death.). It's also revealed that he had a relationship with Dr. Gina Atwater at some unspecified time, but a remark from the latter that their romance finished after a sudden change in Strange's personality implies that relationship occurred while April was still alive.
Years later, while Stephen Strange was driving to work, he unintentionally witnessed the battle between a group of mages and a large, tiger-like demon, despite them all having used magic to become invisible, much to the mages astonishment (mainly Wong). Though shocked, Strange dismissed this as a stress induced hallucination and head back to the hospital, where he treats the other employers with cold arrogance. While in his office, the hospital's administrator Dr. Oliver informed that Gina's having problems and successfully convinced a reluctant Stephen to take a look. When Stephen arrived at the clinic, he become annoyed over the fact the the patient is a child, but Dr. Atwater reveals that her condition is much more grave then what it appears to be and that several other children have become in coma and she didn't know what to do. When he tried to examine the young girl, she suddenly grabbed his arm and Strange has a vision of a "burning face", and when the parents confirmed that the child has that same vision, a scared Strange leaves, claiming that's nothing that he can do help, despite Atwater's protests. While Stephen drives away, he has more demoniac visions and his attempt detour from ghostly children leads Strange to suffer a car accident and become unsconsious.
When he wakes up and starts to recover, Dr. Gina Atwater reluctantly shows to him his medical chart that indicates that his hands where damaged beyond repair. Despite Atwater's hopes that the diagnostic isn't definitive, Strange soon finds out that his hands barely works, making it hard to hold objects and to even use knobs. Distressed, Stephen used almost all his fortune in experts around the world to fix his hands, but they didn't know how to help him. To make matters worse, his damaged hands prevented Stephen to write checks and access his money, which eventually leads to his cars being repossessed and being evicted from his apartment. Homeless and hopeless, Stephen attempts suicide, but Wong intervenes and gives him a map that would led him to find help in Tibet. With nothing to lose, Stephen convinced a reluctant Gina to pay his flight to Tibet, where Strange makes a his way to his destiny in the icy mountains. He soon loses his map, but successfully found a monastery where he's greeted by Wong and see the Ancient one and his gruffy apprentice Mordo. Do his dismay, the Ancient One claimed that he didn't have the cure for his condition, but rather, he himself must find one by working in the monastery, where he's forced to sleep in a small empty cell.
Despite his confusion over the Ancient One powers and the hard work that he has to do (mostly wich he has to used his crippled hands), Stephen proved to be a dedicated and even competent servant and is daily reward with some mobile in his room for his chores. His patience become tested, however, when he is ordered to destroy a wall within the monastery and take its wreckage away, a task that become progressively harder with the wall being reconstructed the next day with heavier and larger blocks, much to Stephen's chagrin. When Strange asked to Wong how this would cure his hands, He replied that this has nothing to do with his hands, wich leads to a disgusted Stephen to leave the monastery, despite the extreme icy and wild climate from the Tibetian mountains. Until that point in the movie, Flashbacks of Strage's past where revealed to the viewers, and when Stephen remenbers the death of his sister, the Ancient One appears in his mind and both consoled Stephen for his loss while also scolded him for his pettiness, claiming that while he has the natural tendency to help and heals others, not all can be saved, wich means that he isn't responsible for his sister's death and that he should look to the future rather than mourn the past. Stephen accepted the council and returned, but when he asked how he could move the heavy blocks, the Ancient One introduces Stephen to the art of sorcery and explain that, in basis, all is a illusion that can be manipulated, like lifting a heavy object or creating weapons from thin air. Confused but accepting the explination, Stephen has his hands restored and begins his training at the mystical arts.
During his training, Mordo attempted to kill Strange due to Ancient One favoring Stephen over him as the new Sorcerer Supreme and the fact that Stephen accidently humiliated him due to his natural affiniation to magic, but Wong intervened and take on Stephen's training, in wich Strange learned fast to fight and use magic. Eventually, after having completed his trainning and changed his appearance, Strange accompained the Ancient One and his disciples to the nexus between realities, where the Ancient One explained that all the demon attacks happening recently is a warming that the true treat, Dormammu, is coming to their reality somehow. Recognising the demon to be the same that appears in the dreams of the comatose children, Ancient One realizes the Dormammu is using the mind of children to bypass the seals that safe the planet from him, wich leads to Stephen and Mordo volunters to rescue the children from Dormammu's reach. After putting Gina to sleep, Strange start to wake the children one-by-one, but unfortunately, Mordo secretly make a deal with Dormammu and assaulted Stephen in the back. Fortunately, Wong once again intervened and saved Strange's life.
Dormammu soon possessed the comatose children, and to Strange's distress, he soon find out that all the other diciples are death and the Ancient One was killed by Mordo. Wong then offered the Ancient One's Eye of Agamotto and explained that, before his death, the Ancient One chosed him to be the new Sorcerer Supreme. Despite feeling unworthy for the title, Strange accepted the position knowing that no one else could. While Dormammu slowly entered the reality through the children's mouth, Doctor strange confronted Mordo and easily defeats him. However, it was too late and Dormammu soon devoured Mordo for his incompetence and forcibly take the eye of Agamotto, allowing Dormammu to become more powerful than ever. Stephen engages a brief but intense mystical battle against Dormammu, but when it becomes clear that the demon had the upper-hand, Wong informed to Strange that Dormammu is made of pure magic, and that his own power can used against him. Strange takes advantage of this knowledge and, to Wong's and Dormammu's suprise, not only he managed to get the eye back, but also use it to completly absorve the demon's essence within it, leading to Dormammu's physical body to crumble to dust. Wong praises Strange, but warmed that still exist several malevolent supernatural beings in the universe and that they still have a lot of work to do. In the next morning, Stephen magically appears in the mind of Dr. Gina Atwater to thanks her and visits his sister's grave, finally comming in terms with her death. As they enter in the Sanctum Sanctorum, Wong reveals that they already are looking for new diciples and cassually mentions a woman named Clea as a powerful canditate.
