History
The Red Skull is Captain America's son. On the night before Steve Rogers, a.k.a Captain America was going to set off to war, he and Gail Richards, his girlfriend, had one last night together. It was revealed that Gail was pregnant, and she got the news after she and everyone else had heard that Captain America was 'dead'. The government decided it was best to keep the son a secret, and they took the son into foster care, training him to become a super-soldier, and soon he was stronger than his father. At 17, the unnamed son figured he was strong enough, and ready, and he killed all the doctor's and soldiers at the facility. He even went into a kitchen, took a knife, and carved his face to near bone, hence, the Red Skull. After decades of being a professional assassin, The Red Skull was working for A.I.M. After the Ultimatum wave was over, and things got back into place, The Skull and other soldiers decided to use the opportunity to rob from the Baxter building, which was momentarily unsecured. They stole some sort of weapon that was hazardous to the world. Captain America and fellow member Hawkeye attacked and possibly killed all the A.I.M. soldiers, and then Cap met his match, as the Red Skull attacked, and nearly killed Cap very easily. Red Skull told Cap he was his son, and then threw Cap out of the helicopter.

Notes
The Red Skull made his first appearance in Ultimate Comics: Avengers #1. His origin was revealed in issue 2. The reason he has a red skull is because he used a knife to peel the skin, cartilage, and fats off his entire head, only leaving the muscular appearance.
Trivia
With several flashbacks of Captain America's past, no mention was given of a son. It was revealed not even Captain America knew he had a son.
